About FRNSW forms
Fire and Rescue NSW (FRNSW) use either Microsoft Word or Adobe PDF forms which include enhanced functionality to work as electronic forms, and should be opened in their native application. Completed forms are to be emailed to firesafety@fire.nsw.gov.au.
When using FRNSW forms for any application or process, please fill in all applicable sections as required. We recommend that you review the information in your completed form then save it. Ensure that you have all required supporting documents or information ready to submit, then email the completed form and required documents to the Fire Safety Branch email address (firesafety@fire.nsw.gov.au).
Microsoft Word forms
All Word forms should be opened using Microsoft Word. Word forms utilise content controls for entering or selecting data (e.g. text fields, drop-down lists, date pick selector), and the appearance of these content controls may change when used. Only enter information into the content controls where provided (i.e. do not add or remove any form content).
These forms may also include macros coded in Visual Basic to improve the functionality of the form, and as such the file may have a ‘dotm’ file extension. A typical example is when being asked how many issues there are, the macro will generate that many number of entries within the form.
Depending on your security settings, you may be prompted by a security window advising you to disable any macro. If you encounter this security message when opening the form, FRNSW recommends you enable the macro to retain the intended functions. If you disable the macro these functions won't function and you will need to manually add content.
The email system in use may also prevent you from sending an email with a 'dotm' file attachment. Please save this file as a 'docx' format document before sending.
PDF forms
All PDF electronic forms should be opened using Adobe software such as Acrobat, Acrobat Reader DC or Document Cloud. Adobe Acrobat Reader DC is available for free download at www.adobe.com
PDF is an open source file format independent of software, hardware or operating system. However, PDF electronic forms are propriety owned by Adobe Systems and enabled through user extension rights. Third-party software, viewers or web browser plugins may not be compatible with PDF electronic forms and the form may not function correctly. It is recommended that you either disable any third-party software/viewer/plugin that is set as default, or right-click to save the form then open in its native application.
Note: FRNSW takes no responsibility for customers who experience issues with electronic forms using software other than approved Adobe software.
